Baptisms at Mt. Carmel Shrine

Requirements:
· Photocopy of Child’s Birth Certificate
· Photocopy of Marriage Contract
· Permission letter from home parish for non-parishioners
· Application Form from the Office

For COMMUNITY BAPTISMS

Every Sunday 2:00 P.M.
Baptism will be held inside Mt. Carmel Church

Fees:
· P 100.00 – Per Sponsor
· P 400.00 – Registration, Panyal (white garment) from the Office, Certificate, Candle for Parents, 2 Candles per sponsor (in excess of sponsors provide their own candles)

NOTE: Requirements and Fees must be accomplished ONE WEEK before Baptism

For INDIVIDUAL BAPTISMS

Fees :
· P 2,000.00 – Venue/Stipend for Priest
· P 100.00 – Per Sponsor
· P 300.00 – Panyal (white garment) from the Office, Certificate, Candle for Parents, 2 Candles per sponsor (in excess of sponsors provide their own candles – IN GLASS)

Monday to Saturday Schedule
9:00 a.m.; 10:00 a.m.;
11:00 a.m.; 3:00 p.m. and 4:00 p.m.
Sunday Schedule – 9:00 a.m.; 10:00 a.m. and 11:00 a.m.

Baptism will be held at the the Pilgrim Chapel of the St. Joseph Building

Reservation is FIRST COME, FIRST SERVED.
Requirements and fees must be submitted and settled ONE WEEK before Baptism.

Please be punctual, only one (1) hour is allotted for the entire baptism.

Baptismal Certificates –

to procure a copy of your baptismal certificates, please provide us with the following information

Full name
Date of Baptism

Please wire or deposit Philippine Peso 500.00 to have a copy of the baptismal certificate sent to a Philippine address, or U.S. Dollars 40.00 to have a copy of the baptismal certificate sent to an address outside of the Philippines

Scheduling a Baptism –

Please finalize the details of your Baptism through email, and wire or deposit the amount needed to the appropriate bank account
